GUIDELINES FOR THE REGISTRATION, QUALIFYING EXAMINATION, INTERVIEW AND RESERVATION
GRADE ONE applicants will take the ENTRANCE TEST which will be given on the scheduled date. They must have the following qualifications:
1. 6 yrs. & 6 mos. of age by June of the school
year. 2. Pre-elem. education 3. Physically fit
For GRADES TWO - SIX applicants:
1. A conduct mark of at least 80% or its equivalent.
2. A passing grade in all subjects in all grading periods and should have consistently maintained a
general average NOT LESS THAN 80% in all grading periods.
3. Physically fit.
For FIRST YEAR HIGH SCHOOL applicants will take the ENTRANCE TEST which will be given on the
scheduled date. They must have the following qualifications:
1. A satisfactory conduct mark.
2. A passing grade in all subjects in all grading periods and should have consistently maintained a
general average NOT LESS THAN 80% in all grading periods.
3. Physically fit to study in a TECHNICAL SCHOOL.

REQUIREMENTS:
The following must be complied with to facilitate the registration.
1. Photocopy of the APPLICANT'S BAPTISMAL CERTIFICATE
2. Photocopy of the APPLICANT'S BIRTH CERTIFICATE (from the National Statistics Office)
3. Photocopy of the APPLICANT'S REPORT CARD (Form 138) with complete information of the first
and second grading periods (with an average of 80% and above)
4. Two (2) recent 1x1 pictures (colored or black & white).
REGISTRATION PROCEDURE:
1.
Proceed to the Registrar's office with all the required documents. Those with incomplete
documents will not be entertained or allowed to register.
2.
Fill out the registration form.
3.
Submit the form to the Records' Check-In-Charge for approval.
4. Proceed to the Cashier's office to pay for the Entrance Examination fee.
5.
Attach one picture on the registration form and another on the receipt.
Return the accomplished registration form to
the Registrar's Office.
VERY IMPORTANT:
1.
The receipt of payment with the attached picture will serve as an ADMISSION SLIP on the day of
the examination.
2.
On examination day, make sure you have these: admission slip, pencil, and snacks.
3.
Take note of the following:
Assembly time, place & the test result schedule from the Registrar's Calendar.
(Only applicants whose names appear in the list are
qualified for the interview)
RESERVATION:
Reminders for Parents
1.
If your son is included in the final list, please DO NOT FAIL to make your reservation. Proceed immediately to the Accounting Office for the payment of the reservation fee.
2.
Failure to do so would mean that you are not interested in enrolling your son. Your son's slot will be given to other qualified applicants.
3. The reservation fee for incoming First Year High School is PHP 1,000.00 (Non-refundable) upon cancellation but deductible from the tuition fee.
Note: Reservation fee applies to the incoming first year high school only.
